Information you choose to share
When you contact us, you may provide your name, work email, company, role, phone number, locations, and a short description of the support you need. The form also records the request type, source page, any supplied campaign reference, and the version of this notice you acknowledged. Please send a business-level description rather than employee files, medical information, incident reports, credentials, or other confidential records.
How your inquiry is used
Your submitted details are used to route and respond to your inquiry, understand the service or demonstration you are interested in, and discuss a possible engagement. The first page you visited, referral category and any supplied campaign references can accompany your inquiry to help us understand how you found us. The form is not a newsletter signup. Optional advertising measurement requires a separate choice; it is not required to send an inquiry.
Website improvement and inquiry sources
We count clicks to inquiry forms, the first interaction with a form, and whether the delivery service accepts or fails to accept a submitted request. These first-party application events contain a known website page, referral category and request type; delivery events also include service and timing categories if you supplied them. They exclude names, email addresses, company names, phone numbers, form messages, raw campaign text, full referring URLs and visitor identifiers. Hosting providers may separately process technical request information, including IP addresses, to operate and protect the service. Application-event history depends on our hosting provider's log retention and export settings; it is not a permanent visitor history.
Optional Google measurement and paid-search inquiries
The EHS support landing pages ask for your name, company, email, state and support interest. Phone, timing, location count and brief business context are optional. Your inquiry and its reference are delivered through Resend to info@prevent-x.co and hosted by Vercel. The email includes a server-issued inquiry timestamp and any permitted campaign context. A short-lived signed confirmation cookie contains a random reference, not your contact details, and confirms an accepted submission. Sending the form requests a response about your business need; it does not sign you up for unrelated marketing.
Your Google privacy choice
Google privacy preferences are separate from ChatGPT advertising preferences. Analytics permission allows us to retain the first paid landing page and supplied UTM campaign fields in this browser tab for up to 24 hours. Advertising measurement permission also allows Google Ads to measure accepted inquiries, use advertising cookies and process technical browser information and permitted click references for attribution. GCLID, GBRAID or WBRAID references may accompany your inquiry. These fields are visitor-supplied attribution context. Withdrawing advertising permission removes click identifiers from the active source record and clears accessible Google advertising cookies. Analytics continues only if you keep analytics permission. Rejecting all optional measurement stops our collection in both categories. Global Privacy Control and Do Not Track override an earlier choice. Your preference is stored for up to 180 days; the introduction of native Google Ads measurement requires a new choice. The form remains usable without optional tracking or browser storage.
What measurement includes
With analytics permission, the paid funnel records landing views, CTA clicks, form starts, validation or delivery errors and successful submissions. Google Analytics receives these events only when configured. With separate advertising measurement permission, Google Ads receives a conversion only after a validated request is accepted by the email delivery service and the server marks it eligible. Known internal addresses and explicitly marked tests are excluded. Opening a thank-you page is not a conversion. Events use a permitted page path and random duplicate-protection reference; analytics may also include service category and state. Names, emails, phone numbers, company names, free-text answers and raw click references are excluded from our event payloads. No contact hashes are sent. Google tags load only with permission on the six paid EHS pages. Google ad personalization is denied; Enhanced Conversions contact matching and automatic sales-stage uploads are disabled. Inquiry acceptance is not proof of sales qualification or a signed customer agreement.
Optional ChatGPT advertising measurement
If you allow advertising measurement, the OpenAI pixel runs on selected public service and inquiry pages to measure ads and completed inquiries. We send a lead-created event only after the delivery service accepts your form, with a random submission reference for duplicate protection. OpenAI also processes technical and browser information, permitted page and referrer information, and advertising identifiers. Automatic advanced matching can detect supported email addresses and phone numbers on these pages, normalize and hash them in your browser, and send the hashes to OpenAI for matching. Supported geographic details may be normalized and sent without hashing. Hashing does not make matching data anonymous. The pixel can detect supported fields while you interact with a consented page, before a lead event. We do not add your name, message or contact details to our manually constructed conversion event. We send the pixel's opt-out flag for future user-level ad personalization; this does not disable automatic advanced matching or conversion attribution.
Website and email processing
Inquiry details are sent to our team at info@prevent-x.co through the website's email delivery service. Hosting, delivery, and mailbox providers process information needed to operate those services. Technical request information may also be used to protect the website from abuse. A successful form confirmation means the delivery service accepted the request; it is not confirmation that someone has read it. If the form cannot send, you can contact us directly by email.
Browser storage, tools and external links
Session storage can keep your first website page, referral category and supplied campaign references in this browser tab so they survive navigation to an inquiry form. This source record expires after 24 hours and is normally cleared when the tab session ends. Your optional advertising choice is stored for 180 days. With permission, OpenAI may use a click-reference cookie (__oppref) for up to 30 days and a browser-identifier cookie (__obref) for up to 365 days, plus consent storage, a single click reference retaining its original 30-day expiry, and a bounded record of submitted conversion IDs for 30 days. If storage is unavailable, the form still works. Browser Do Not Track and Global Privacy Control signals disable client event collection. The safety starting-point tool keeps its answers in your browser and does not submit them to PreventX. The platform demonstration uses fictional records. External websites have their own privacy notices. Reject optional measurement or withdraw permission at any time through Cookie preferences in the footer; this stops future OpenAI measurement and clears our advertising references in this browser. Browser privacy signals override an earlier advertising choice.

Describe your request with only the information needed to identify the relevant conversation. Any rights and obligations depend on the applicable law and circumstances; we may need to verify a request before disclosing or changing personal information.
